We enable people to live as independently as possible through comprehensive and flexible support in the community and residence of their choice. We use a person-centered philosophy to develop a program that is unique to the supported person. We currently serve Delaware (New Castle County).

It promotes wellness, safety, and independence through personalized planning, appointment management, medication oversight, and proactive health monitoring.
Community Access Support
Community Access Support empowers individuals with mental and physical challenges to safely engage in social, educational, and recreational community activities. It promotes independence, inclusion, and confidence through personalized planning, skill-building, and supportive guidance.
Local Services Advocacy
Local Services Advocacy empowers individuals with mental and physical challenges to access and navigate community services, fostering independence and self-advocacy. It provides personalized guidance, representation, and education to help residents assert their rights, secure supports, and fully participate in community life.
